Kane County Housing Continuum of Care
The Kane County Continuum of Care’s mission is to end homelessness in Kane County.
Established in 1999, the Continuum brings together local governments and social service agencies to support individuals and families who are homeless or at risk. Partners provide outreach, assessment, shelter, permanent housing, and supportive services, working collectively to identify and address service gaps.
The Continuum secures approximately $1.5 million annually through HUD’s Continuum of Care grant process, helping fund essential programs and expand affordable housing options across Kane County.

You're Invited: Kane County Human Services Emergency Readiness Exercise

This interactive working session will bring together human service organizations across Kane County to begin building a coordinated Human Services Emergency Readiness Network. Participants will strengthen relationships, identify collective capabilities and resources, and prepare to respond more effectively as community needs evolve.
Kane County has extraordinary organizations serving marginalized, disadvantaged, and vulnerable residents. But changing funding, policies, program requirements and growing demand are challenges no single organization can address alone. Every organization has a role. Whether you serve 10 people or 10,000, your experience and capabilities matter.
Through facilitated discussion and realistic scenarios, participants will identify resources, build connections, and develop action steps for stronger regional coordination during emergencies and periods of increased need. This session is the first step in an ongoing effort that will continue through future exercises and collaborative planning. Register today.

Coordinated Entry System (CES)
The Coordinated Entry System (CES) connects people who are experiencing homelessness to housing opportunities in Kane County. The CES uses a standardized housing assessment and follows a prioritization plan to refer youth, individuals, and families to housing. The assessment is available to all eligible persons regardless of race, color, national origin, religion, sex, age, familial status, disability, actual or perceived sexual orientation, gender identify, or marital status.
Point In Time Surveys
The Point-in-Time (PIT) Count is a count of sheltered and unsheltered people experiencing homelessness on a single night in January. HUD requires that CoCs conduct an annual count of people experiencing homelessness who are sheltered in emergency shelter, transitional housing, and Safe Havens on a single night. CoCs also must conduct a count of unsheltered people experiencing homelessness every other year (odd numbered years). Each count is planned, coordinated, and carried out locally.
